Job Description

American Traveler is hiring an LPN for a skilled Home Health assignment requiring 1+ year of Home Health experience and proficiency in HCHB documentation.

Job Details

• Skilled Home Health setting with a 1:1 patient ratio,
• Up to 6 patient visits per day with approximately 30 productivity points per week,
• Day shift schedule, 5x8 hours (8:00 AM-4:30 PM),
• Weekend support may be required,
• On-call rotation may be required, to be discussed during interview,
• Service area may extend up to approximately 60 miles or 60-90 minutes of total daily driving,

Job Requirements

• Active Oregon LPN license required before start date; temporary licenses not accepted (license required if applicable),
• Minimum 1 year of skilled Home Health experience required,
• BLS certification required,
• Proficiency in Home Care Home Base (HCHB) required,
• OASIS experience required for staff completing Start of Care (SOC),
• Reliable personal transportation and valid driver's license required,
• Skills required include medication administration (oral, inhaled, subcutaneous, IM/SQ, ear/eye drops), NG/G tube care, and wound care/wound vac,
• Knowledge of infection prevention practices required,

Additional Information

• Responsibilities include resumption of care and routine home visits,
• Documentation and communication must be completed in a timely manner,
• Utilizes Remote Client for Home Health on laptop and Rover phone application,
• Business casual attire required; scrubs permitted but not mandatory,
• Free parking available, and mileage reimbursed at current IRS standard rate,
• Up to 3 days of RTO may be automatically approved; blackout periods include Thanksgiving, Christmas, and New Year's weeks,
• First-time travelers are not accepted for this role,
• Local candidates accepted within a 50-75 mile radius at the same pay rate,
• Former permanent employees must be separated for at least 6 months and approved by HR before returning as a traveler,
• Travel pairs accepted, but each traveler must have their own territory and personal transportation,
• Monday orientation required, including completion of billable training modules
